📋 Documents Required for Claim

  • Personal Application for NOK (Hand Written)
  • Photocopy of Death Certificate
  • Photocopy of Aadhar Card of NOK
  • Photocopy of PPO(Pension Payment Order/Discharge Book where of NOK Mention
  • Surrender Liquor card with application
  • Bank Details of NOK
  • A completed application/claim form your Dependant Canteen

📝 Steps to Claim ADLRS Support

  1. Report the death immediately to the nearest Station Headquarters or CSD Canteen (URC).
  2. CSD/Station field staff will assist in organizing the funeral or take full charge if no family is present.
  3. The wreath is arranged, and documentation is submitted for financial disbursement.
  4. The grant is paid to the family/NOK either directly or via the canteen/station office.
  5. Reimbursement is handled internally by the URC and doesn’t require follow-up from the family.
